Insurance Important Notes

  • Your insurance card and valid QID must always be submitted when availing treatment.
  • Valid guardian’s QID will be necessary for the minor patients when availing treatment.
  • Your signature on the claim form is mandatory, approval and insurance invoice are necessary when availing treatment.
  • Read your insurance policy and benefits carefully.
  • Some insurance policies include patient’s deductible either fixed amount or a fixed percentage.
  • Some insurance may not be available in our clinic.
  • Some insurance policies may not be eligible to avail treatment in our facility.
  • Some procedures may not be covered when availing treatment.
  • Copayments will be based on patients’ insurance Table of Benefits.
  • Any uncovered services must be paid by cash and cannot be reimbursed.
  • In the case of partially approved treatment, the uncovered amount must be paid in cash by the patient in addition to the co-payments if applicable.
